SUMMARY

Transitioning from residential care to family-based care is a difficult process for any organization. CAFO studied several organizations that have successfully transitioned and were willing to share their experiences.

While each organization and mission vary, they share common characteristics that may be adapted to other settings, situations and cultures. These case studies are intended to help organizations take the next steps toward family-based care.

HOW TO USE

These case studies are intended to offer encouragement and ideas to help similar organizations begin the process of transitioning toward family-based care. Each of these is a model, with characteristics that may be considered within your own context.

This is not intended to serve as a step-by-step guide for transitioning organizations. Contact information for each studied organization is available within this publication, and CAFO encourages organizations to reach out for further discussion and guidance along your journey toward family-based care.
